Chances are you will have a back problem at some point in your life. Understand that pain is localized to a specific area has tentacles that reach the brain in the form of nerves. When are nerve is traumatized certain events take place to resolve the ensuing tissue changes. You may have pain in all phases of your recovery. If you understand what takes place in each phase of back pain you will have a better idea of how to navigate through to fully resolve the back pain.
Say you hurt your back skiing. Before that, you would notice an occasional twinge after playing golf. Maybe you had to take a few days off from work, an Advil every now and then. For argument’s sake, there was something already going on. You may have skied thirty times already that season, but this one run really did your back in. Now, you have bad back pain. Let’s get under the hood
In Stage 1, your tissue around the joint you injured is going to be inflammed. Motion is lost and you can expect pain, swelling, and muscle spasm. Swelling and inflammation is the same thing. It will last up to 72 hours. All you want to do here is rest and support the area and apply ice. Do this for no more than 3 days.
Somewhere between 2 and 4 days, fluid will become congested in the tissue. This adds to the restricted motion and will delay healing. What tissue are we talking about? The muscles primarily with the swelling but the ligaments that attach bone to bone are restricted in their ability to move. Lack of movement is a bad thing. Your body is headed in the wrong direction. To effect the change you would want to remove the fluids to get some pain relief. This is where an anti-inflammatory comes in handy. Slowly, you would want to start stretching lightly. You are only a few days out from the injury.
Anywhere from 5 days to 6 weeks, you are entering into the repair phase of your injury. You might be pain-free at this time but ignoring the fact that you had an injury can have dire consequences. You want to avoid the formation of scar tissue. I prefer to call scar tissue adhesions. Excess scar tissue formation leads to a disruption of normal function. This is usually a jumping off point for many people with back pain. They have no pain and feel they are better. Maybe if you are young and in great shape, for the rest of us humans, this presents a problem.
Adhesions that restrict motion leads to chronic stiffness, accelerated degeneration, besides chronic back pain and weakness. So if you decide to drop out of care it is like skipping dessert. This is the best and probably the most important part of healing a bad back. Repair or despair. The idea is that if motion is restored and maintained your damaged tissues heal in a more functional manner yielding lasting results.
Let’s get to the end stage of your injury to the remodeling phase. Just by completing this phase of care you will find that you are at reduced risk of re-injury and can ward off degeneration. By remodeling damaged tissue you will have better strength and flexibility. On a side note, this could take up to a year depending on the severity of the tissue damage.
So what are your takeaways? Understand that back pain is self-limiting in back injuries. It is that last thing to show up and the first thing to go away. The underlying tissue needs time to heal properly. Work with your doctor to find out where you are in different stages of care.
